Public Notaries Appointment Rules (NSW) – Form 5 – Notification of Change of Particulars

Under Rule 10 of the New South Wales Public Notaries Appointment Rules, any changes to the particulars of the Notary Public is to be communicated to the Registrar at the Legal Profession Admission Board. The notification is done by way of a From 5, which is contained at the end of the Rules, in Schedule One. The Form 5 must be completed with all the new and updated particulars within one month of those changes taking effect. The particulars which the Registrar will be interested in include the name of the Notary Public, his address, the name of the firm, the telephone and fax numbers at which the notary can be contacted as well as the DX (document exchange) number.
